When a card shows two suit icons, does it count twice for suit-based scoring?

Yes. Suit-based scoring counts icons rather than cards, so a card showing both suits (a City, or a region with two matching icons) counts once for each icon, for example 2 VP for Boats on a card with both grain and water.
