If an Attack or Defense upgrade roll fails, must I spend the point elsewhere?

Yes. A failed Atk or Def upgrade roll doesn't cost the Training Point; you must spend it on something else, such as HP, Dex or a Skill. Once both Atk and Def attempts have failed, those options are gone for that point.
