The rulebook gives the per-day penalty; players, including the German editor, say it starts that same day, as a Chapter 1 reminder card shows.

Immediately: you lose 2 Health and gain 2 Terror at the same Start of the Day the last Menhir goes out, and again at each Start of the Day until you die or activate a Menhir. The last Location cards stay on the map. When any other Location is discarded, those on it take the 2 Health and 2 Terror right away.
