How many chapters does the campaign have, and can any be skipped?

There are 15 chapters, and two of them have alternate versions of which only one is played in a given run. Depending on earlier choices, some chapters (for example 7 and 12) can be skipped. The main story has no overall time limit, though some side quests do.
