When must a card that adds damage or Accuracy be used during an attack?

By step 4, Apply Modifiers. Effects that add dice must come right before the roll, rerolls happen in step 3, and effects that gain or remove icons or Accuracy belong to step 4, so playing a modifier after surges are spent (for example at damage calculation) is too late to add damage or Accuracy.
