How do the achievement sheet and unlocked quest cards work, and what if the rulebook and the sheet disagree?

The sheet is one Achievements/Totem list; each totem or ending counts once across all campaigns, and the running total unlocks quest cards at set thresholds (for example 4 unlocks quests 171 and 172, with 169 and 170 coming at 7). Use the numbers printed on the sheet: the rulebook's picture and numbers are outdated. Only the Unlocked Cards carry into a new campaign, and you may choose to start with fewer quests.
