The designer said item effects apply while held and spend is a controlled term.

Yes. An item's effect works as long as you hold it and stops when you no longer have it. Only effects whose text says to spend the card use it up, and spend is always optional. Gear and black-background Advantages are ongoing and are never used up by use.
