How does time work while the stairs card is visible?

Every time spent for any reason (start of turn, consequences, costs) goes on the stairs as a damage token instead of discarding a card; each set of three removes the tokens and deals one damage to a hero, which may repeat. You can stay on the floor as long as cards remain, and with no cards left time goes to the stairs too.
