Before the encounter, roll one or two black dice; if either shows a 1 you take the listed penalty (damage and time) just once, even if both show 1. The dice then count for the encounter, the penalty resolves immediately and cannot be avoided by a reroll, and the feat is usable in any non-boss encounter.
