The Lord Ruler draws after each player's turn, so a freshly drawn Adversary doesn't hit you right away. Adversaries with an end-of-turn effect trigger at the end of the turn of the player they are in front of, before that player draws their hand; others are permanent effects. In co-op he gets a turn after every player's turn.
