Are treatment rooms limited to a set number per player?

No fixed per-player limit: all treatment rooms are laid out at setup and can be built freely, up to the 16 tiles in the box; the practical limit is how many supply rooms and how much space you have. A clarification file's mention of four per player was meant for supply rooms, and that limit applies only to setup.
