Can staff or patients leave my clinic?
The rulebook says people cannot leave; the designer confirmed doctors leave only under an expansion rule, and the orderly case is a community reading.

No. Once someone has moved onto your board, the only way out is for a patient to be cured or to die. Unpaid staff don't leave and you can't dismiss staff; the rulebook's remark about clearing parking by not paying staff is regarded by players as a mistake. Players report one exception, following the original rules: an orderly with no free supply room is removed along with their car. Some expansions add their own exits, such as workaholic doctors leaving after treating.
