Inside the Clinic
Forget everything you've learned about Clinic through all the expansions. With this expansion, your goal is not to treat patients, nor even sustain them; rather, you strive merely to satisfy your most primal instincts….
Setup
You start the game with more staff in your Clinic…. Unfortunately, they are not here to help you treat patients, but to have fun and sex with each other and with the patients, doctors, nurses, and orderlies you admit/hire during the game: Each player takes 2 lustful doctors, 1 lubricious nurse, and 1 seductive secretary. They are waiting at the entrance to your Clinic. Don't forget to add their 4 cars!
Each player also takes 4 Restroom tiles. One starts built along with the 3 other tiles from the base game. Keep the last 3 near your player board to build later. The Restroom tile must be adjacent to at least 1 of the 3 tiles built during setup on the ground floor.
You also start the game with more money: $30.
Each player also puts his Lovemaking marker on 0 on the Popularity track. Stack them in the same order as your Popularity discs.
Tip: I do not recommend playing this expansion on the 3×3 side of your Clinic boards, because you always start with an additional 4 meeples in your Clinic, which means 4 more cars than usual….
Playing the Game
Phase 1: Actions
Action 1: Build
The Restroom tile is a new module; it respects all the usual building rules. You can only have 1 Restroom tile per floor. 2 Restroom tiles cannot be adjacent (so not even above/below each other).
If you decide to build a restroom during your Build Action, it does not count as one of your component builds (it's a "bonus build"). It costs $1 + $f where f is the floor on which you build it.
Move
Lustful doctors, lubricious nurses, and seductive secretaries only go to the restroom, and no other modules. You can only have 2 of them in each restroom. If you don't have enough restrooms, the rest wait lasciviously at the entrance.
New Restroom capacity:
- a total of 2 lustful doctors, lubricious nurses, and/or seductive secretaries AND
- a total of 2 patients, doctors, nurses, orderlies, janitors, pregnant women, temps, physiotherapists, assistant nurses, lively elders, visitors, workaholic doctors, CEO, surgeons, secretaries, stretcher bearer teams, pop stars, bariatric doctor, plastic surgeon, gynecologist, genetic counselor.
Example: 2 lustful doctors) and (1 nurse + 1 visitor).
NEW! How to Have Sex in the Clinic
- Lustful doctors only make love with sexually active patients.
- Lubricious nurses only make love with non-lustful doctors.
- Seductive secretaries want to make love with non-predatory, sexually active people: anyone except babies, burn victims, critical patients, elders, and zombies.
Lustful doctors, lubricious nurses, and seductive secretaries do not make love to each other; they are far too predatory for that level of consent.
Move people who are already in your Clinic (not in the Pre-admissions areas, nor at Entrances/Helipads) to restrooms, respecting the Restroom tile capacity. They don't spend any time getting there… with great gusto.
Having sex in a restroom takes 2 time (just a quickie between shifts).
Advance your Lovemaking marker on the Popularity track 1 space for each sexual encounter that just occurred in your clinic's restrooms.
The sex partners stay in the restrooms for the rest of the round (quick and neat are mutually exclusive).
Phase 2: Business
Patient Care and Income
All people who had sex during this round have no other function, desire, treatment, or motivation to move. They stay in the restroom until the end of the round.
Expenses
Restroom upkeep is $1. If some people had sex in a restroom during this round, orderlies must clean these restrooms (one could do them all, or you can have a different orderly for each, or any combination): Choose an orderly to move from their supply room to each used restroom you wish them to clean, and then back to their original supply room, spending the usual time for travel, as if it were the Move phase. Spend 2 time in each restroom to clean up. Do this with each orderly you wish to utilize until all used restrooms are clean.
Orderlies who made love in the restroom are not available to perform the upkeep, because they're more focused on performing other "duties". It must be an orderly who is in a supply room.
If you don't have any orderlies to clean the restrooms, restroom upkeep is $6 (you have to call in specialists once things are much crustier). If you have an orderly, you cannot choose to pay $6 instead, just to avoid spending time, but you can choose which orderly will clean up the restrooms if you have several options.
Pay your employees
Lustful doctors, lubricious nurses, and seductive secretaries cheerily and salaciously do not expect any salary; they are only in your clinic for sexual conquests. Consequently, their cars never leave your clinic.
You must still pay the salaries of the people who are shirking their duties in the restroom….
Phase 3: Admin
Return people who had restroom sex to any modules you wish in your Clinic for 0 time (they sneak back when no one is looking).
Example: Patients can move to different treatment rooms than where they started. It's the magic of love!
Patients still worsen in the restrooms (Is there a better way to bid this world goodbye?), and doctors definitely still level down from exhaustion.
End of the Game
The player whose clinic hosted the most sexual encounters during the game (Lovemaking marker is the highest on the Popularity track) wins. In case of tie, the tied player with the most Popularity wins. If still tied, the tied player who reached that particular Popularity first wins (as in the base game).
1 Restroom tile scores for 1 Sexual relation. 2 Restroom tile scores for 3 Sexual relations. 3 Restroom tile scores for 6 Sexual relations. 4 Restroom tile scores for 10 Sexual relations. Move your lovemaking marker on the track accordingly.
Gynecologists
Nothing off color here; just adding our gynecologist friends, who had not yet been represented in the game. You will finally be able to hire your favorite gynecologist — perhaps not as lewd as you'd imagined, ya perv!
Note from the designer: This expansion cannot be combined with Obstetrics from The Extension, nor can it be combined with Pediatrics from 2nd Extension because pregnant women and babies are handled completely differently in this expansion.
Setup
Place the Specialty Pharmacy board near the main board and fill it with 4 Contraceptive Pill tiles for each player.
Place p + 1 pregnant women and Baby Patient cubes on the Specialty Pharmacy tile, where p is the number of players (e.g. in a 3p game, 4 pregnant women).
Playing the Game
Phase 1: Actions
Action 2: Hire
When you choose this action, if there is at least one white doctor at the university, you may hire a gynecologist if you don't already have one in your clinic. The gynecologist costs double the most expensive white doctor available at the university (which could be $0). Of course, the gynecologist comes with a car.
Action 3: Admit Patients
You can now use one QP to move a pregnant woman from the specialty pharmacy to an entrance/helipad. Don't forget to add a car!
Move
The gynecologist will only move to (and treat patients in) a treatment room with 2 adjacent supply rooms.
If the Gynecologist moves to such a treatment room, the capacity of that treatment room is modified as follows:
- 1 gynecologist and 1 patient or
- 1 gynecologist and 1 pregnant woman
The patients a gynecologist can treat include the following: normal white, yellow, orange, and red patients, Clinic addict (YOU) if you're female, pop stars (but only if the gynecologist sells them a contraceptive), pregnant women, sexual obsession patients, and unclassified patients.
Phase 2: Business
Patient Care and Income
Each gynecologist can only examine 1 patient or 1 Pregnant woman in a treatment room, with 0 nurses present.
If the gynecologist treats a patient, they sell the patient a Contraceptive Pill tile and you earn $12. Take a contraceptive pill from from the top to the bottom of the the specialty pharmacy and tuck it under the patient. (The source prints "from from" and "the the" here; they are transcribed as printed.)
If the gynecologist treats a pregnant woman, a new Baby Patient cube is born: Replace the pregnant woman with a white patient cube and add a pink Baby cube from the supply to the same module: You earn $6 + 3 popularity. Remove the pregnant woman from the game (she's a white patient now). Don't remove the car, it belongs to the white patient (new mother) now!
1 nurse and 1 orderly in a functional treatment room can treat a baby. You earn $25. A Baby counts like a normal patient toward the capacity of a treatment room. When the baby is treated, do not remove a car.
Treatment Room Capacity:
- 2 patients (a baby is a patient) + 2 doctors + ∞ nurses or
- 2 babies + 2 nurses + 2 Orderlies or
- 1 gynecologist + either 1 pregnant woman or 1 patient
Pay Your Employees
The gynecologist gets a $1 salary.
End of the Game
Score negative popularity if you still have contraceptive pills in the specialty pharmacy, according to the scoring scheme on the Specialty Pharmacy board.
Obesity Institute
Obesity — whether via genetics, eating disorder, dissolute lifestyle, or a combination of those, or even something else — consumes a larger and larger percentage of the population. As the director of the most profitable clinic in the region, you understand that it is your profit responsibility to help these good people. The Obesity Institute should help tip the scales toward fitness.
Setup
Place 4 morbidly obese patients per player near the main board; return the rest to the box.
Park 4 SUVs per player (1 per morbidly obese patient in the game) in the parking lot on the Obesity Institute board.
Place 1 bariatric doctor per player above the University on the main board; return the rest to the box.
Place 1 Bariatric service hub per player near the main board; return the rest to the box.
Place 1 morbidly obese patient on the leftmost space of each open service queue on the notepad of the main board. It's fine if there are gaps between them and the rest of the queue.
Note: Setup refers to the "bariatric doctor"; the rest of the rulebook calls this piece the bariatric surgeon. It is the same component.
Playing the Game
Phase 1: Actions
Action 1: Build
You can build a Bariatric service hub. For your VIP and snobby patients, the morbidly obese patients are not very popular; that means you cannot build the Bariatric service hub on Floor 0–1.
In order for morbidly obese patients to use your conveyor system, at least 1 module in the conveyor network must have 2 conveyors in it. This upgrades the whole network.
Action 2: Hire
When you choose this action, if there are yellow doctors at the University, you may hire a bariatric surgeon if you don't already have one. The bariatric surgeon costs double the most expensive yellow doctor at the university. As usual, this person comes with a car!
Action 3: Admit Patients
Morbidly obese patients always wait at the leftmost space of each queue; they do not advance other than by you spending QP. So, to admit a morbidly obese patient, you must spend the QP to make them the rightmost patient in a service queue; they go from there to pre-admissions as usual.
Each morbidly obese patient arrives in an SUV that must be parked with an appropriate display of privilege: It must span two parking spaces. Simply park it "diagonally" across the corner of the parallelogram space between the two parking spaces the SUV occupies. Until the SUV leaves, neither of those parking spaces is available.
Diagram caption: If you need to park a new car, you cannot use parking space (a) or (b), because someone rudely parked their SUV across both parking spaces.
If you park the SUV in a parking lot, of course straddle it over 2 parking spaces, preferably diagonally (to mimic real life, and to help you remember that it's blocking both spaces).
New Patients Schedule Appointments
Add patients to the queues as usual, except that morbidly obese patients there cheerily stand still on the leftmost space, inviting newcomers to pass them.
Move
Morbidly obese patients move like everyone else in your Clinic, except:
- They need twice the time, so they spend 2 time per space, rather than 1.
- They can only use a conveyor network that has a double conveyor at one of its stops (i.e. 2 conveyors on the same module). Then it's 0 time between conveyors, just as it is for everyone else.
Stretcher bearer teams are not capable of moving morbidly obese patients.
Morbidly obese patients can only be treated in a treatment room adjacent to a Bariatric service hub. If you admit a morbidly obese patient (for some reason) before you have a functional Bariatric treatment room, the patient can wait at any service hub.
Treatment Room Capacity: 1 morbidly obese patient + 1 bariatric surgeon (or the usual other options).
Phase 2: Business
Patient Care and Income
One morbidly obese patient can only be treated by one bariatric surgeon in a functional treatment room with no other patients, that is adjacent to the Bariatric service hub. After treatment, place the morbidly obese patient on the Obesity Institute board and remove 1 SUV, freeing 2 parking spaces. You earn $20, modified by the usual treatment bonuses (gardens, satellite TV, air conditioning…).
Expenses
Each untreated morbidly obese patient in your Clinic costs you $1.
Pay Your Employees
Your Bariatric Surgeon gets a salary of $2. If you don't want to pay, they return to the top of the University.
Buy Popularity
After everyone has bought their popularity, then in reverse turn order, each untreated morbidly obese patient in your Clinic earns you 3 Popularity.
Phase 3: Admin
Morbidly obese patients already on the notepad move one space right like the other patients, but they are never removed when they're on the rightmost space with the red [icon not reproduced in the text layer].
For the service queues that no longer have morbidly obese patients, add one from the first 4 × p morbidly obese patients at The Obesity Institute, where p is the number of players.
Clinic Randomizer
Anatomy of a card. Each card depicts:
- the components required
- a colored disc that identifies which box it comes from (orange for the base game, red for The Extension, blue for 2nd Extension, green for 3rd Extension, and so on…). If there are two discs, you will use both!
With more than 60 expansion modules (if you have the base game, The Extension, 2nd Extension, 3rd Extension, 4th Extension, 5th Extension, 6th Extension, and these pink and blue 69th Extensions… plus Biohazard and COVID-19), it might be difficult to choose among all of them (the Campaign Book can be a great guide, but it doesn't include these latest releases). This deck of cards will save you a lot of time in choosing a random subset.
Setup
Before setup, agree on the number of cards you will include, and draw that many random Module cards. We recommend not using more than 5 cards, to reduce the interaction between the expansions; 4 is a good number. If you are not happy with a card, just discard it and draw another one. Variety and fun are the purpose of this "expansion", after all!
Simply take the required components from the indicated boxes, and read the corresponding rules from its rulebook. Play, and that's it!
If you have the other randomizer cards from 6th Extension, you can combine both decks of cards.
Cosmetic Surgery
With such popularity, your clinic is becoming world famous! As a result, special requests are coming in from some high-profile faces. It's time to offer a new specialty: cosmetic surgery. Stars around the world will adore you! But be careful not to mar that moneymaker: A star is forever illuminated by the spotlight!
Setup
Your clinic is now committed to offering cosmetic surgery. That means each player starts the game with an operating room that must be built during setup. You also start the game with a plastic surgeon at the entrance, with a [normal] car, of course.
Place the VIP Parking board below the Parking area of the main board. Populate it with limousines and pop stars as indicated on it, per the player count (each player has their own VIP parking lot).
Example: In a 3-player game, there will be 12 limos, 3 white pop stars, 3 yellow pop stars, 3 orange pop stars, and 3 red pop stars.
Playing the Game
Phase 1: Actions
Action 3: Admit Patients
You can spend 1 QP to move a pop star from your VIP parking to any entrance/helipad in your clinic.
Move
In this expansion, the operating room is dedicated exclusively to treating pop stars. Pop stars and plastic surgeons move to operating rooms.
Operating Room Capacity: 1 pop star + 1 plastic surgeon.
Phase 2: Business
Patient Care and Income
1 plastic surgeon can treat 1 pop star in an operating room. Plastic surgery is tricky business, and a pop star's face gets way more scrutiny than most: You must see whether the results please the pop star: Draw a cube from the Patient bag:
- If the drawn cube matches the pop star's color, the pop star is satisfied: Either earn 5 popularity or immediately decide to use this rate during the Buy Popularity phase (for this round only): Each popularity costs $4.
- If the drawn cube mismatches the pop star's color, the pop star is dissatisfied, and publicly shamed: Either lose 5 popularity (min. 0 on the track) or immediately decide to use this rate during the Buy Popularity phase (for this round only): Each popularity costs $2.
Diagram: a matching cube gives 5 popularity or buys popularity at $4 each (=, "5 / -$4: 1"); a mismatching cube loses 5 popularity or buys popularity at $2 each (≠, "5 / -$2: 1").
Expenses
Each limousine that is still waiting in the VIP parking lot for its pop star pays $4 for premium parking. This is shared among all players, rounded down. There's just the one place to pay, so everyone splits the profits.
Pay Your Employees
The plastic surgeon gets a salary of $8.
Buy Popularity
Depending on your decision after treating a pop star, buy popularity at either the usual or special costs. If you did not treat any pop stars, use the usual cost.
Phase 3: Admin
Remove treated pop stars from the game, along with their limousines!
End of the Game
−5 popularity for each pop star still in your VIP parking.
