Does Khorne get a counter for each kill?

No. One counter per region per phase where he kills, so kills in different regions each give one, but extra kills in the same region do not. A second battle (Reborn in Blood) in the same phase and region doesn't add a counter. He double-ticks only if he strictly has the most counters.
