A Bake Bread action activates every baking improvement you own, so you can combine their rates (Clay Oven plus Cooking Hearth turns the first grain into 5 food). Fireplace and Cooking Hearth have no limit on grain per action, the Clay Oven handles one grain per bake and the Stone Oven two. Building an oven gives an immediate Bake Bread action. You can only bake in a Bake Bread action; at any other time grain simply converts 1-for-1.
