Do Mobsters already on the board, or those from setup, attack again each Act?

No. A Mobster attacks only once, when it enters Manhattan in the Mob War at the end of Act 1, 2 or 3. Setup Mobsters never attack, and Speakeasies you take over from older Mobsters are not attacked later.
