Yes. An outbreak removes a Supply cube from or places a Plague cube in every adjacent location, and havens are locations, so a connected haven is affected. Players generally rule that a haven that would get a fourth Plague cube does nothing, since only cities outbreak, but the rulebook does not say so explicitly.
