Does an outbreak happen at 3 cubes in total, or when a colour would get a 4th cube?
The Outbreaks section says up to 3 cubes of each colour.

Only when a 4th cube of one colour would be added. A city can hold up to 3 cubes of each colour, and outbreaks can leave several colours in one city.
