Does a permanent defence token give a token immediately, and does it come back each chapter?
The rulebook's Permanent Tokens section answers this directly.

Yes to both. You get the matching Combat Token at the start of every chapter. If you earn the permanent token before the Encounter part of a chapter, you also gain the token immediately for that chapter. You gain it once per chapter, not again between story and encounter.
