When your Oxygen reaches 0 you gain the token and the dial resets to 0. While you have it, the next time you lose Health you die. Discard it when you gain Oxygen or end your Turn in a Section with Active Life Support. Oxygen is lost only in a Section with Inactive Life Support, and you can drop to 0 and then lose again.
