Only a Metagorger attack gives you a Mutation card (if you have none), plus a Contamination card and 1 Light Wound; other Carnomorphs attack and wound you normally. Mutation markers come from using your Mutation Action (which Scans a Contamination card from your hand) or from an INFECTED Scan, which gives a marker instead of a Larva. At 4 markers you die. Markers stay all game; Surgery or the Antidote removes only the Mutation card.
