A bare-hands or improvised Melee Attack first draws a Contamination card, and a miss costs you a Serious Wound. The Wrench and Crowbar are Item Actions, not Melee or Shoot: you roll the Combat die and deal Injuries as for a Shoot, but with no Contamination card, no Advantage die and no Serious Wound for a miss.
