Whenever the card is revealed. The old rulebook text ties them to the staging step of the quest phase, but the FAQ extended them to any reveal, so a surge card revealed in setup or by a card effect still draws another card. A card that is merely added to the staging area, put into play or attached has not been revealed, so no When Revealed effect, Surge or Doomed applies.
