Setup follows the General Player Aid and each class sets up at the same time, read as two columns: Working Class and Capitalist Class first, then Middle Class and State. The Middle Class is meant to know the Capitalist Class's companies before choosing its starting worker. In later Preparation Phases, any timing conflict is resolved in normal turn order, so the Working Class draws its immigrants before the Middle Class picks its skilled worker.
