The attack example image on page 22 of my rulebook shows a Bloodlust icon before the attack. Is that right?

No — the text is correct and the picture is a printing error in the newer rulebook printing: the Killer targets and attacks with no Bloodlust increase in that step. Earlier rulebook versions have the correct image.
