If I am insane, when do I make a sanity check?
The rulebook states the timing of the sanity check explicitly.

Once, at the start of each of your turns before you draw. It is not triggered each time an Other World card is drawn or played. You stay Insane for the rest of the round, so you check every turn, revealing as many cards as you have Other World cards in your discard zone.
