When must Caprice Nisei be rezzed for her psi game to trigger?

Before the Runner passes the last ICE: rez her during the approach window (step 2.3) of the last ICE, or before the run starts if the server has no ICE. Rezzing at step 4.3, after the last ICE, is too late, and there is no rez window during an encounter. Replies disagree about edge cases such as the last ICE being trashed mid-approach, and no designer post settles it.
