Adjacency (a shared hex side or a bridge) decides which buildings form a city and when neighbours gain power. Reach, which adds river hexes up to your Sailing value, decides where you may build and how groups are counted at the end of the game. Building in reach but not adjacent starts a separate cluster, which is how you can found several cities.
